| What to Expect During a Visit to Our Office |

the patient is a key ingredient in determining an accurate diagnosis. The better prepared
you are to provide this information, the better we will be able to serve you. |
| Our Evaluation Process |

During your initial
visit you will be given a detailed health questionnaire to complete. Dr. Kunkle will ask
you about your medical history and your symptoms. A focused orthopedic examination will
then be made by Dr. Kunkle. |

While many patients want
to discuss in great detail the exact event which may have caused their injury, the most
important information we need in evaluating your condition involves your symptoms (such as
where you have pain, weakness, numbness, etc.) and what limitations these symptoms place
on your everyday activities. You can use the form described above to help you accurately
prepare this information prior to your visit with the doctor. |

After reviewing your
history and diagnostic tests, you will be presented with a number of treatment options. We
will inform you of the risks and benefits of each option and we will assist you in making
the final decision concerning your treatment options. However, it is important for you to
understand that the final decision about which treatment option is best for you rests
solely in your hands. |
| On the Day of Your Visit |

On your initial visit,
please bring with you any requested items (such as X-rays, CT or MRI scans, etc.) as well
as information concerning your insurance, Workers Compensation or Medicare coverage. |

Please be aware that
occasionally the appointment schedule may be disrupted by our need to attend to a
seriously ill patient. Your understanding in these situations is appreciated. |
| Business Matters |
|
You are responsible for
paying for the initial visit at the time of your visit. If you have any questions about
your insurance coverage, including Medicare, please ask a member of our office staff. |
| In An Emergency |
|
If you have an emergency
after normal office hours, you should call our office and your call will be
forwarded to Dr. Kunkle. |
